They are on an emergency medical call; the doctor is summoned to an Indian camp, where a pregnant Indian woman is having a very difficult birth.

The first real experience of life that is given to Nick during the emergency medical call to the Indian Camp, is as they arrive to the hut where the pregnant woman is lying, the place is not at all what you would call suitable for a birth. ... The problem is that this is not a rich country like Denmark, this is a small Indian camp, where there are no possibilities of bringing the woman to a hospital.
