The gym is where Phy Ed is held and during the lunch hour the gym turns into the “Star Rock Café.” The main office is where the nurse’s office, principle’s office, and the secretaries are. Around the outside are all the classrooms and the library which connects the school so you can walk all the way around it without having to go back around. 2. Personal Data: Age, Sex, Parents, Siblings: Peter is a 6 year old boy who has lived in Rochester his entire life. He has a mom and dad and one younger brother. As I can tell they are middle or high class with all the important things a child at this age would need. 3. History: Family life, where the client has lived, factors leading to involvement with the agency. Peter has a very normal life as I can tell, probably with more than just the necessities. Peter seems really normal when you first see him, maybe just extra shy compared to the rest of the children, but I think there might be something more. He doesn’t have any learning disabilities, is a little bit slow but nothing to be worried about. He’s not allergic to anything, he doesn’t hit or bite. Just a very normal first grade boy. 4. Client behavior: Mention things you feel are typical of the person. Mention anything that makes your client special to you. I do homework with Peter a lot because he is really slow and sometimes doesn’t fully understand the directions. Its not like he is mentally slow he just goes slow on all the cutting and gluing that goes on in the first grade. When I work with Peter one on one he is the most amazing little guy. Turns out he loves art so I think that’s why he goes so slow because he wants to do a good and thorough job. His work never turns out all that great though. He also is a very slow reader, he knows the words he just is slow at remembering them. When we read together he loves to joke around, I can see him growing up and being quite hilarious. He always makes funny voices with different characters and some days he just uses a funny voice all together. I guess I have learned to get use to this and just let him be creative when he wants to be but the way he acts in class makes him look like he may have a learning disability. 5. Problems/ needs: Describe specific needs of the client as determined by the and/ or the CAP volunteer. Peter just needs some extra attention that is the main thing he needs to help him succeed but he also needs to know that attention won’t always be there and he needs to become a more independent worker. He needs special help with some of his homework, for example he needs to be reassured that he is doing everything correctly. He’s not as sure of himself as he needs to be. He should just cut and color and be happy with the final result but he always asks if he is doing it right and when he knows he is then he feels free to be creative the way he wants. So he knows what to do he just always needs reassurance. He’s also very shy in class; he never raises his hand, even if he has the right answer.
